Mommy and Me Nutrition Care Program™ Scholarship Application

Are you a pregnant mama looking to nourish your body and your baby without feeling overwhelmed?
The Mother-Baby Nutrition Foundation is proud to offer scholarships to the Mommy and Me Nutrition Care Program™.
The Mommy and Me Nutrition Care Program™ is lead by a Registered Dietitian Nutritionist who specializes in perinatal nutrition and is available to any and all women who are trying to conceive, pregnant, and postpartum.
However, our foundation understands that finances can be a burden for some women, and we want to make sure all moms have the opportunity to learn how to better nourish themselves and their little ones regardless of their financial situation.
That is why we are proud to offer scholarships to moms that are either on state medicaid or uninsured to participate in the Mommy and Me Nutrition Care Program™.

To Qualify For The Scholarship
We have 3 scholarships available.
- You must currently be pregnant.
- Because of our current funding source, only Tennessee residents are able to apply for the scholarship at this time. So you must live in the state of Tennessee to qualify for the scholarship.
- You must either be on TennCare (state Medicaid) or uninsured.
- You must download and fill out the application via the secure link below.
